T&T: health insurance

I just got on Medicare and, with a supplemental policy, I have greatly cut my
health care costs. Looking to reduce my (much younger, alas) wife's costs as
well. We are nearing our liveaboard-here life to liveaboard- cruising
transition, so I would appreciate any information on or off-line about
listees' approach to health insurance. It's hard to have a gatekeeper
physician when you're never where he is.
